Notice of Privacy Practices

Effective date: April 21, 2025. This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment to your privacy

At Tennessee Premier Health, we are committed to protecting your personal health information. This notice describes your rights and our legal duties regarding your Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA).

How we may use and disclose your health information

Treatment
We may share your health information with other providers to coordinate and manage your care.
Payment
We may use and disclose PHI to obtain payment from insurance companies or third-party payers.
Healthcare Operations
We may use your PHI for administrative purposes such as quality improvement, training, accreditation, and compliance activities.
Appointment Reminders and Follow-Up
We may contact you by phone, text, email, or other methods to remind you of appointments or provide follow-up care information.
As Required by Law
We may disclose your PHI when required by federal, state, or local law.
Public Health Activities
We may report information to public health authorities to prevent or control disease, report child abuse, or ensure FDA compliance.
Law Enforcement
We may share PHI with law enforcement when legally required, such as in response to a court order or subpoena.

To Avert a Serious Threat

Other uses and disclosures require your authorization

For all other uses or disclosures not covered by this notice, we will obtain your written authorization. This includes marketing communications not related to your care, sale of your PHI, and most uses of psychotherapy notes (if applicable). You may revoke your authorization at any time in writing.

Your rights regarding your health information

  • Access Your PHI: Request and receive a copy of your medical records.
  • Amend Your Records: Request corrections to your health information.
  • Request Confidential Communications: Ask us to contact you in a specific way (e.g., home phone, mail, email).
  • Request Restrictions: Ask us not to share certain information. We may not be able to accommodate all requests.
  • Receive a List of Disclosures: Ask for a list of the disclosures we have made of your PHI (excluding treatment, payment, and healthcare operations).
  • Receive a Paper Copy of This Notice: Even if you agreed to receive it electronically.

Complaints

If you believe your privacy rights have been violated, you can file a complaint with Tennessee Premier Health (phone (865) 234-4395, email support@tnpremierhealth.com, address 145 Bear Crossing, Ste. 100, Office 112, Mount Juliet, TN 37122) or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.

Changes to this notice

We reserve the right to change this notice at any time. Updated notices will be posted on our website and made available upon request.
