Individual Adult Membership
$100/month
Designed for adults who want consistent, relationship-based primary care without the complexity of traditional insurance billing, this membership provides direct access to your provider when you need it. It includes unlimited telehealth visits, up to four in-office visits per year, same- or next-day scheduling, secure messaging, chronic disease management, preventive care, medication management, care coordination, and discounted lab pricing. This plan is ideal for individuals who value accessibility, continuity, and personalized care.
Family Add-On
$75/month per additional adult
$40/month per child
Family maximum: $300/month

Additional adults and children receive the same access benefits, including telehealth visits, in-office visit allowances, preventive care, chronic condition management, and messaging access.
